Development of a Program Maturation Model Using a Multiple Case Study Approach

Abstract: The Centers for Disease Control and Prevention (CDC) has funded five sites to implement colorectal cancer screening among uninsured, low-income populations. These five sites, together, comprise a demonstration project for which the aim is to examine various service delivery models in order to explore the feasibility of establishing a national colorectal cancer screening program. As part of a multi-method evaluation effort, a multiple case study is being conducted to understand and document program implementation processes over a three-year period. Anticipating the need to guide programmatic diffusion, the case study team conducted a cross-site qualitative analysis with the intention of describing hallmark characteristics, activities, successes, and challenges of programs at various stages of maturation. This paper presents a working theory of program development and maturation based on an evaluation of implementation processes. Findings will be used by CDC to develop and provide technical assistance to future colorectal cancer screening programs.
A Qualitative Evaluation Component of a Healthcare-based, Multi-year Quality Improvement Initiative

Abstract: In May 2007, Nemours Health and Prevention Services (NHPS) began a quality improvement initiative (QII) among a sample of primary care pediatric clinic sites (n=11) and school wellness centers (n=4) in Delaware. The purpose of this two year initiative was to determine if quality improvement strategies, education, and tools could improve primary care provider care and outcomes related to childhood overweight. As part of the comprehensive evaluation, in April 2008 (Year 1) a series of focus groups (n=4) was conducted so that participating physicians and staff could provide NHPS with critical feedback on QII components, materials, and tools. In Year 2, a second round of focus groups (n=4) will be conducted by June 2009, so to determine sustainability and effectiveness of Year 1 changes to QII components. Results from both rounds of focus groups, along with implications for improvement of the QII will be presented.
